Loan Request and Adjustment Form

A. Before submitting this form, please make sure the following have been completed:

• You have been admitted to the college in an approved major/program.
• You have completed a valid FAFSA and submitted all requested information, if applicable.
• You are making Satisfactory Academic Progress
• You have completed your online Loan Entrance Counseling
• You have completed your Master Promissory Note
• You are enrolled in at least 6 credit hours

B. Student Loan Information 

The chart below illustrates the amount of Federal Direct Student Loans you may receive annually/over a lifetime. Please review this chart and information below before submitting this form to avoid delays in the processing of your loan request. • Federal Regulations require that all Federal Direct Loans be issued in two equal disbursements. If the loan covers two semesters within the academic year, loan funds will be disbursed in equal amounts each semester. If the loan covers one semester onlu, loan funds will be disbursed in two seperate and equal amounts in the same semester.
• Amounts awarded will be determined by need and budget as well as the amount requested on this form.
• Exit Counseling is required, if you fall below 6 credit hours, withdraw, transfer, or graduate.
