  • INSTRUCTIONS FOR COMPLETING THE APPLICATION: Complete the entire application and click SUBMIT, or save your application and complete it later by selecting the SAVE button.  If you choose to save and complete later, you will receive an email from noreply@formresponse.com with a link to return to your application. Always use this email to return to the existing application.  Do not start a new application. Once the SUBMIT button is clicked, you cannot return to the application.

Once the minimum required information has been entered, the full application will become visible.  

    PROGRAM INFORMATION

    The Practical Nursing Program is designed to prepare the student to meet the licensure requirements for Licensed Practical Nurse (LPN), as established by the Louisiana State Board of Practical Nurse Examiners (LSBPNE). The program progresses from simple to complex and consists of classroom instruction, lab practicum and supervised clinical activities in accredited hospitals, nursing homes, and other health care agencies. Upon graduation, the student is awarded a diploma and is eligible to apply for the National Council of State Boards Licensure Examination for Practical Nurses (NCLEX-PN).

    National Council Licensure Examination (NCLEX-PN)

    The NCLEX is a test that is administered to all Practical Nursing (PN) graduates. It is meant to ensure that each nurse is competent and can practice safely as a new nurse. The test is based on Computerized Adaptive Testing (CAT). CAT basically means that each test is different. Each candidate receives an individualized and unique testing experience. The NCLEX exams are produced by the National Council of State Boards of Nursing (NCSBN). Each year, the NCSBN surveys newly licensed nurses in order to update and ensure relevancy for the NCLEX. This ensures that the test matches up with what nurses are really doing in their field. The content for the NCLEX-PN falls into 4 main categories with several smaller subcategories. All test content is based on client needs. Nurses must use their knowledge of medical procedures, assessment, and more to care for the client in the best way possible. Students will receive their license from LSBPNE upon completion of NCLEX-PN.

  • CURRENT ENROLLMENT STATUS

  • Applicants to the Practical Nursing program must have submitted an online application for admission to SoLAcc for the semester they wish to begin OR be a current student at SoLAcc.

    To complete an online application for admission to SoLAcc, visit www.solacc.edu/ApplyNow.  Applicants will receive a Student ID Number via email.  If you currently have classes scheduled, you do not need to complete a new application.

  • MINIMUM TEST SCORES

  • ONLY INFORMATION FOUND ON THE ACADEMIC PROFILE WILL BE CONSIDERED.

    Applicants must provide the highest Accuplacer NG, Accuplacer Classic, or ACT test scores taken within three years of the program start.  The English, math and reading scores can be entered from different types of tests with different test dates.  Information for the minimum test scores must be copied from the Academic Profile in LoLA.

    You MUST REVIEW the Practical Nursing Packet to see if you need to report the completion of Math 0083 or English 0083.

    • Select Prior Education and Testing from the menu.

     

  • Enter Math Test Information

    The minimum acceptable math scores are ACT Math of 19, Accuplacer NG Algebra QAS of 250, or TEAS Math score of 62

  • You have not earned a minimum math score required to apply.  Please refer to the PN packet for more information about the minimum required test scores.  

  • Enter Reading Test Information

    The minimum acceptable reading scores are ACT Reading of 20, Accuplacer NG Reading of 250, or TEAS Reading of 62.

  • You have not earned a minimum reading test score required to apply.  Please refer to the PN packet for more information about the minimum required test scores.

  • Enter Language Test Information

    The minimum acceptable language scores are ACT English of 18, Accuplacer NG Writing of 250, or TEAS English 62.

  • You have not earned a minimum language score required to apply. Please refer to the PN packet for more information about the minimum required test scores.

  • SoLAcc CUMULATIVE GPA

  • ONLY INFORMATION FOUND ON THE ACADEMIC TRANSCRIPT WILL BE CONSIDERED.

    Students who have taken classes with SoLAcc must enter their cumulative institution GPA.  This value must be copied from the student profile in LoLA.  Only students who have taken classes at SoLAcc will have an institution GPA.

    To view the institution GPA in LoLA:

    • Login to LoLA
    • Select STUDENT under South Louisiana Community College in the Self Service Banner box.
    • Under Academic Links, select Academic Profile.
    • Select Curriculum and Courses from the menu.
    • On the Curriculum and Courses page, select Hours & GPA.
    • Provide the Institution GPA value and hours.

     

  • ACADEMIC HISTORY

  • ONLY INFORMATION FOUND ON THE ACADEMIC TRANSCRIPT WILL BE CONSIDERED.

    Applicants to the practical nursing program will receive additional consideration for early completion of coursework that demonstrates academic strength.  These courses are not required; however, successful completion of the courses will improve an applicant's selection score.  Enter the highest grade completed for the following courses.  Pay close attention to when the course(s) must have been completed.

    Information for the prerequisite courses must be copied from the academic profile in LoLA. Applicants must have earned the grade specified for each of the courses.

    • Select Academic Profile under South Louisiana Community College, in the Academic Links box.
    • Select Academic Transcript from the menu.

    All transcripts from previous colleges must be sent electronically to the Admissions Office at SoLAcc.  Applicants will be notified via email when a transcript has been evaluated.  Transcripts from out-of-state colleges can take up to one month to be evaluated.

    If applicants have questions about transfer courses not appearing on their academic profile, they should email SLCCtranscripts@solacc.edu.

    This is a 3-credit course that must have been completed with a C or higher.  Enter the following information from the academic transcript in LoLA.

    This is a 3-credit course that must have been completed with a C or higher within 3 years of submitting this application.  Enter the following information from the academic transcript in LoLA.
